I have a therapy agreement and data consent form which I will send you prior to your initial appointment and which I will ask you to complete, sign and return to me following our initial appointment if you choose to continue working with me after our first meeting. These outline how confidentiality and privacy are maintained within my therapy practice and within the therapeutic relationship itself.
